What is Title VI? The Civil Rights Act of 1964
§2000d Prohibition against exclusion from participation in, denial of benefits of, and discrimination under federally assisted programs on ground of race, color, or national origin
No person in the United States shall, on the ground of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving Federal financial assistance.
(Section 7101 ). It is the purpose of this part to support the efforts of local educational agencies, Indian tribes and organizations, postsecondary institutions, and other entities to meet the unique educational and culturally related academic needs of American Indian and Alaska Native students so that such students can meet the same challenging State student academic achievement standards as all other students are expected to meet.

Indian Student Eligibility Certification Form for Title VI
All students who have tribal membership in a federally recognized tribe or who have a parent or grandparent with tribal membership are eligible to receive support services under the Title VI grant through SPS Native American Education Department.
This federal program ensures that schools provide a quality education for Native students required by tribal treaties. Even if you do not anticipate utilizing Title VI services, filling out an ED 506 form for your eligible student will increase the grant funds available to all Native students and their families who may be in need of support.
Please complete the online form below to enroll in Title VI or Opt Out. Your information is confidential and will remain secure within the Native Education Department and will not be shared without your consent. The form only needs to be filled out one time and will remain with your student while enrolled in SPS.
A paper form is still available for those unable to complete the online option. To complete the paper form, print out, fill in and sign, and return it to your student’s school.
